HOW TO BOOK
There are two convenient ways to book a virtual appointment with the Pearson Health team:
Referral from your Healthcare Practitioner: To provide continuity of care, we prefer that you obtain a referral from your physician or nurse practitioner, if possible. Please ask them to fill out our Practitioner Referral Form and fax it to our clinic at 844-240-9273.
Self-Referral: Self-Referrals for a number of qualifying conditions are also available. Please download and complete our Self-Referral Form and e-mail or fax it to our office. You may also call us at 519-339-9233 to discuss this option further.
Once the referral form has been reviewed by our practitioners, you will be contacted to book an appointment.
